I have been learning about this stuff recently (I'm a US citizen, my fiance isn't) and what I understand is:
To enter the US with a work-related visa, you must be sponsored by a company in the US. This sponsorship must be agreed to and the paperwork submitted during one of the two times of year in which work visas are reviewed - one is the month of April, the other is the month of October. You can contact companies via email / internet to send them your cv and let them know of your interest or can visit them in the states (provided you have the appropriate visitor's visa if you are coming from a country that doesn't allow the typical 90-day period in which you can visit without a visa. Essentially, you can't get the work visa unless you have a corporate sponsor and it's on you to find that corporate sponsor. I would suggest starting with some internet-based searches, because there are a lot of companies that advertise their willingness to sponsor non-US workers.
